Ask for decisions, action items and open questions as separate groups. Keep a suggestion out of the decisions list unless the notes show agreement. Do not assign an owner just because someone mentioned the task.
Compare each action with the source notes. Mark a missing owner or due date as unassigned. When a transcript contains timestamps, keep them beside disputed or unclear items.
Only after checking the facts, shorten the language for the people who need it. Use a writing assistant to improve clarity without changing who promised what. Ask attendees to resolve any remaining gaps.
